Sign into your Paycom Employee Self-Service

Click on Employee Self-Service ESS. Navigate to the Payroll tab, and select Year-End Tax Forms. There you can view, download and/or print your document.

We are located in the Frye Building on 21st Avenue in Forest Grove. Office hours are 9 a.m. to 4 p.m Monday, Tuesday, Thursday, & Friday (Appointments are appreciated). The physical office is closed on Wednesdays, but staff are available via email.
