NOTICE OF PRIVACY PRACTICES

Reporting of Unauthorized Access to Protected Health Information

Any member of the Pacific community (staff, students, faculty, etc.) including business associates must notify the Pacific University Privacy Officer if they become aware of an incident that may involve unauthorized access to Pacific University protected health information. Notification can be provided through the following means by email or phone.
