Flu Shot Clinics | Faculty and Staff

Kaiser Permanente-sponsored flu shot clinics will be on the Hillsboro and Forest Grove campuses in October.

Flu shot clinics are open to faculty and staff employees and their covered family members 9 years and older. Children under 18 must have parental consent.

Printing and completing an intake form prior to your flu shot clinic visit will save time. Please also review the Vaccination Information Statement. Bring your insurance card to the flu shot clinic for review.

Employees do not have to have Kaiser insurance to participate. Visit the Employee Wellness to gain access to accepted insurance carrier information, the intake form, and the VIS.

Forest Grove Campus: Monday, Oct. 25
Pacific Hall - Room 219, 9 a.m. to 11 a.m.

Hillsboro Campus: Tuesday, Oct. 26
HPC 2 - Room 213, 10 a.m. to noon
